Reception hours and non-consultation days
Consultation is unavailable on Saturdays, Sundays, national holidays, and year-end and New Year holidays (December 29 to January 3).

For patients visiting Hokkaido University Hospital (Dental Care Center) for the first time (first-visit patients)
Please come to the Dental Care Center between 8:30am and 12:00pm. Upon arrival, complete the Consultation Application Form with your name, address, and other necessary information in the outpatient ward on the 2nd floor, and bring the completed form to the first-visit reception counter at window No. 1.
Please also present a referral from another medical institution, if you have one.
To receive insurance-covered treatment, you must present your health insurance card.
Special charge for patients without a referral (combined insurance-covered and non-covered medical treatment fee)
For patients visiting our hospital for the first time without a referral, a special fee of 3,240 yen will apply (in addition to the first-visit fee, if applicable).
Patients are strongly encouraged to obtain a referral from another medical institution.
For returning patients
Patients with a Patient Registration Card (ID card) can simply check in using the return visit reception machines located in the outpatient ward on the 1st floor or near the reception counter on the 2nd floor of the Dental Care Center.
If you have an appointment, your check-in will be automatically processed, and a waiting ticket will be issued.
After completing the check-in process, please wait in the waiting room of the outpatient clinic you have come to see.
The dentist will decide based on the results of your dental examination if the first-visit fee should be added to your medical bill.
Please present your health insurance card at the reception counter (window No. 1) at your first visit of every month.
You are also required to do so in the middle of the month if any change has occurred in your health insurance status.
Forgot to bring your Patient Registration Card (ID card)?
Please come to the return visit reception counter (window No. 2) to check in.
Calculation of your medical bill
After the consultation, please come to the payment counter (window No. 5 or 6) on the 2nd floor, where your bill will be calculated. It usually takes 1–2 minutes to calculate your bill.
Paying your medical bill
When your bill is ready, your Patient Registration Card will be returned to you. Please pay using the automated cashier machines located in the outpatient ward of the Dental Care Center.
The machines accept both cash and major credit cards.
The machines are in operation between 8:30am and 5:00pm.

If you have scheduled an appointment for your next visit, you will receive an appointment reminder, regardless of the necessity of payment, from the automated cashier machine.
