Each clinical division has its own non-consultation schedule for weekdays. Please check the outpatient consultation days before your visit.
Visit procedure

For those visiting the Hokkaido University Hospital for the first time, please come between 8:30 am and 12:00 noon. Upon your arrival, please complete all the applicable items on the Consultation Application Form in the outpatient hall on the first floor and submit it at window No. 3, No. 4 or No. 5 at the first-visit reception counter.
At the counter, indicate which clinical division you would like to be seen by. Please also submit any referrals from other medical institutions at this point.
For treatment covered by health insurance, you must present your health insurance card.
Those with Patient Registration Cards (ID cards) seeking a first consultation from a different medical division from the one where they originally registered are regarded as first-visit patients and are requested to repeat the first-visit registration procedure. Please check in at the return visit reception machines in the outpatient hall on the first floor and wait at the first-visit reception counter until your name is called from window No. 3, No. 4 or No. 5.
A first-visit fee applies to patients returning for consultation after an interval of three months or more since their last visit.
If you do not know which medical clinical division you need to visit or you are not sure which division your symptoms apply to, please refer to the information on outpatient consultation days and medical clinical divisions.
If you are currently receiving medical attention at another medical institution, it is highly recommended that you ask your doctor to issue a referral from there.
Please note that we are not able to answer telephone inquiries regarding this matter.
Special charge (combined medical treatment fee not covered by health insurance) for patients without a referral from another medical institution:
For patients visiting the hospital for the first time without a referral from another medical institution (if a first-visit fee is required), a special fee of 2,625 yen for the first consultation applies.
We recommend that you obtain a referral from another medical institution wherever possible.
Return visits
| Those with Patient Registration Cards (ID cards) can simply check in using the return visit reception machines in the outpatient hall on the first floor. A waiting ticket will be automatically issued if you have an appointment. If you do not have an appointment, please select the medical clinical division you wish to visit. A waiting ticket will be issued. After receiving the ticket, please wait to be called in the outpatient waiting room of the relevant medical clinical division. Those seeking consultation for the first time in six months or more since their last visit are requested to complete the administrative procedures for first-visit patients. If such procedures are necessary, this will be indicated on the waiting ticket you receive when checking in. Please wait in front of the first-visit reception counter for your name to be called from window No. 3, No. 4 or No. 5. The doctors will decide if it is necessary to calculate your medical bill with a first-visit fee based on the results of your medical examination. Please present your health insurance card at your first visit of every month. If there are any changes in your health insurance status, please report this at window No. 8 of the reception counter. |

Calculation of your medical bill
After the consultation, please obtain a reception number ticket at window No. 11, No. 12 or No. 13 at the payment counter. It will usually take 10 to 15 minutes to calculate your bill. Please be seated until your reception number appears on the electrical display.
Payment of your medical bill
Please pay your bill using the automatic cashier machines in the outpatient hall on the first floor. The machines accept cash and several credit cards.

Appointment for your next visit
If you make an appointment for your next visit at the end of the consultation, you can receive a sheet indicating your visit schedule along with a detailed statement of your medical bill, a receipt and any other related information from the automatic cashier machine regardless of the necessity of payment.
Use of your Patient Registration Card (ID card)
You will need your Patient Registration Card (ID card) for check-in, blood sampling, radiography examination and payment (at window No. 11, No. 12 and No. 13). Please have your card ready at these times.

If you have any questions or need help, please ask for assistance at the general information desk in the outpatient hall on the first floor. Volunteer staff and staff at the reception desk are also always available to provide assistance.
