New Patients

Hokkaido University Hospital is certified by the Ministry of Health, Labour and Welfare as an advanced treatment hospital.
To promote collaboration with other local medical institutions and ensure the sharing of functions among hospitals and clinics, in accordance with national policy, our hospital employs the new patient appointment/referral system. Patients without an appointment may not see a doctor on the day. Please make an appointment in advance. In principle, a referral letter (medical information) from another medical institution is necessary to visit our hospital. Counters No. 3 to No. 5 for first-visit receptionSome clinical departments accept new patients without a referral letter, but please note that patients then have to pay the following special fee based on the combined insurance-covered and non-covered medical treatment system, in addition to the first-visit fee, at their own expense (not covered by insurance).
Medical clinical division: 5,500 yen (non-taxable case for childbirth: 5,000 yen)
Dental clinical division: 3,300 yen

*The fees changed on April 1, 2016 due to revision of the system (no raise for the dental clinical division)
 

New patient appointment/referral system

With the aim of fulfilling our mission as a local base hospital to provide advanced medical care (advanced treatment hospital) and to shorten waiting time for new patients, our hospital employs the new patient appointment/referral system for all clinical departments. We ask for your understanding and cooperation.

(Medical clinical division) New patient appointment/referral system

In principle, new patients have to bring a referral letter and make a prior appointment.
There are some clinical departments where
  • you can see a doctor without a referral letter,
  • you can see a doctor without an appointment, and
  • you can see a doctor without a referral letter or an appointment.

(Dental clinical division) New patient appointment/referral system

It has been necessary to make an appointment since June 2014. New patients are required to make an appointment before their first visit. On the scheduled day, please complete the registration form with your name, address and other information at general dental reception on the second floor, and submit it at counter No. 1 (first-visit reception) with your appointment confirmation form.
If you have a referral letter from another hospital, please present it.

Bring a referral letter.

In principle, a referral letter (medical information) from another medical institution (regular doctor) is necessary to visit our hospital. Some clinical departments accept new patients without a referral letter, but please note that patients then have to pay the following special fee based on the combined insurance-covered and non-covered medical treatment system, in addition to the first-visit fee, at their own expense (not covered by insurance).

There are two ways to make an appointment.

Two ways: 1.) the patients make an appointment by themselves; 2.) another medical institution makes an appointment on behalf of the patients.

1.When a patient makes an appointment by telephone

The following matters are confirmed when making an appointment by telephone:
Name, gender, date of birth, contact information (address, telephone number), desired clinical department, consultation history at Hokkaido University Hospital, name of referrer (medical institution), address provided in the referral letter

 Please call +81-11-706-7010 for the Department of Rehabilitation and Physical Medicine.
 Regarding the Departments of Nuclear Medicine, Radiation Oncology, and Diagnostic and Interventional Radiology as well as the Clinic of Cancer Gene Diagnosis, please have an appointment made through another medical institution.

Reception and non-consultation days

Reception hours

Reception for first visit  8:30 a.m. – noon
*Patients with an appointment are kindly requested to visit our hospital 30 minutes before the scheduled time to check in.
*Until 10:30 a.m. for the Department of Ophthalmology.
*Until 4 p.m. for the Departments of Preventive Dentistry, Dentistry for Children and Disabled Persons and Orthodontics.

Non-consultation days

Consultations are not available on Saturdays, Sundays, national holidays, or the year-end and New Year holidays (Dec. 29 – Jan. 3).
There may also be non-consultation days, even on weekdays, depending on the clinical department.  
 

Check-in procedure

Medical clinical division

New patients check in at the first-visit reception counters in the hall on the first floor of the outpatient building.

If you have made an appointment through your current medical institution

Submit your referral letter (medical information), appointment form and health insurance card at counter No. 1 (community health coalition reception).

If you have made an appointment by telephone yourself

Fill out the red frame of the registration form and submit it at counter No. 3, 4 or 5 for first-visit reception with your referral letter (medical information), appointment confirmation form or a note with details of the appointment, and your insurance card.

If you visit a clinical department that does not require an appointment or a referral letter

Fill out the red frame of the registration form and submit it at counter No. 3, 4 or 5 for first-visit reception with your health insurance card.

*You will be asked which clinical department you want visit.
*Since patients with an appointment are seen first, you may have to wait longer.
 

Dental clinical division

Reception at the Dental Care CenterNew patients check in at counter No. 1 (first-visit reception) of general dental reception on the second floor of the Dental Care Center.

If you have made an appointment through your current medical institution

Please submit your appointment form, referral letter (medical information) and health insurance card at counter No. 1 (first-visit reception) of general dental reception on the second floor.

If you had made an appointment by telephone yourself

Please submit your registration form, appointment confirmation form and health insurance card at counter No. 1 (first-visit reception) of general dental reception on the second floor.
*You will be asked about your scheduled clinical department and time.
 

Patients seeking a first consultation at a different clinical department

Counters No. 3 to No. 5 for first-visit receptionIt is necessary to take the first-visit reception procedure (referral letter and prior appointment).
On the scheduled day, insert your patient registration card (ID card) into one of the return-visit reception machines (located in the hall on the first floor for the medical clinical division and at general dental reception on the second floor for the dental clinical division) and then proceed to a counter for first-visit reception at the medical or dental clinical division.
*The first-visit fee applies to patients returning for a consultation after an interval of three months or longer.

The general information desk is in the outpatient hall on the first floor. Please ask volunteer or counter staff at the reception counters for assistance.
