Hospital Admission and Discharge

Reservation for hospitalization

If a doctor or a dentist decides that you should be hospitalized based on the consultation, you have to make a reservation for hospitalization.
Please take the procedure at the Inpatient Center, where you will receive a hospitalization warranty form and a hospitalization guide. At that time, your contact information will be confirmed and guidance on hospitalization will be given.
 

Decision on hospitalization

If the day of your admission to the hospital is decided, the employee in charge will call you.
Please prepare a hospitalization warranty form.
You are not allowed to park your car in the hospital parking during hospitalization.
 

Procedure on the day of admission

On the day, please come to the Inpatient Center in the outpatient hall on the first floor of the medical outpatient building.
The things you need for the procedure include your health insurance card, hospitalization warranty form, hospital garment application form, patient registration card and any medicine you are usually taking at home. If you have a publicly funded care recipient certificate for intractable diseases, please present it. There are carts to put your baggage on.

Things to bring at the time of admission

Please bring personal belongings including toiletries, a shaver, tea cup, chopsticks, spoon, towel, bath towel, slippers, cardigan and underwear. Bring only the minimum amount of cash you will use to prevent theft. Automatic teller machines are installed in the hospital (Japan Post Bank, Hokkaido Bank and North Pacific Bank). Bedding is provided. A hospital garment can be rented at 81 yen a day.

If you are hospitalized for an industrial accident, please inform us accordingly at the time of the admission procedure because workers’ compensation benefits are paid only for specified diseases.
Injuries due to a traffic accident are basically not covered by insurance. Please inform us accordingly at the time of the admission procedure.
 

How to spend your time in hospital and rules

Please observe the instructions of doctors, dentists and nurses while you are in the hospital.
Any problematic behavior to other patients in the hospital, including drinking and gambling, is prohibited.
You need the permission of your doctor or dentist to leave the building.
Please use the outlet for any electrical appliances you have. A TV, refrigerator, washing machine and ice are available for use at a fee. At a store in the hospital, food, daily goods, underwear, postcards, a home-delivery parcel service and a fax are also available. There is also a restaurant, a barber and a beauty salon in the hospital.

Wristband

In the hospital, inpatients are required to wear a wristband to prevent misidentification and to ensure safe medical care.
The wristband displays the patent’s name (katakana), hospital ward, ID number and bar code. In principle, patients have to wear it on a wrist or ankle from admission to discharge.
The wristband is used to identify the patient at the time of a test, surgery, injection, etc. Thank you for your cooperation.
If it is inconvenient for you, please ask a nurse.

Meals

Breakfast is served at 8 a.m., lunch at noon, and dinner at 6 p.m.
Special diets are provided based on the symptoms (liquid diet, diabetic diet, infant diet, etc.).
Besides general diets, selective meals are provided in the hospital.
A 24-hour tea server is installed in dayrooms.
 

Attendants

Nurses take care of inpatients. No attendants are necessary. If desired, however, family members can attend the patient with the approval of the doctor.

Before visiting a patient, please inform the nurse at the nurse station.
Please use the dayroom to receive visitors and observe the visiting hours.

Calculation of hospitalization expenses

Medical clinical division

The Diagnosis Procedure Combination (DPC) system, whereby the hospitalization expenses per day are calculated based on the disease, the symptoms and treatment (Diagnosis Procedure Combination), or the piecework payment system, whereby the expenses for each course of medical treatment are calculated, is used to calculate hospitalization expenses for patients admitted to our hospital.
  • Regarding the Diagnosis Procedure Combination (DPC) system, the basic hospitalization expenses, as well as the expenses for tests, diagnostic imaging, medication, injections, treatment and pathological diagnosis are included in the comprehensive hospitalization expenses per day (there are certain exceptions).
    The comprehensive hospitalization expenses, doctors’ technical expenses for surgery, etc. and guidance/management expenses are counted to calculate the final hospitalization expenses.
  • The comprehensive hospitalization expenses per day differ depending on the disease, treatment and the number of days in hospital.
    A certain coefficient is decided by the hospital (coefficient by medical institution). The total medical expenses at our hospital may be different from those calculated based on DPC at another hospital although the same treatment is given for the same disease.
    If the disease name changes during hospitalization (when the disease was determined as a result of tests, etc.), the hospitalization expenses for the determined disease from the first day in the hospital are recalculated based on DPC. If you are hospitalized for a period that straddles different months, any excess or deficiency in expenses for the month of admission will be adjusted by incorporating it into the expenses for the month of discharge.

Calculation of hospitalization expenses

Dental clinical division

Estimated hospitalization expenses are calculated at the medical affairs section  (dental clinical division). If you have any questions, please contact the Inpatient Center.
 

Payment of hospitalization expenses

A hospitalization bill for one month is delivered around the 12th day of the following month. Please pay the bill by the end of that month. If you leave the hospital in the middle of the month, please pay the bill before the day of discharge.
You can pay your bill in cash or credit card at an automated cashier machine in the outpatient hall. If you use a credit card, but the bill may exceed your set limit, please make an adjustment with the credit card company to assure smooth settlement.
(Estimated hospitalization expenses are calculated at the Inpatient Center for the medical clinical division and at the medical affairs section (dental clinical division) for the dental clinical division. If you have any questions, please contact the Inpatient Center.)

You can make inquiries about payment for medical care at counter No. 12 for payment consultation in the outpatient hall on the first floor and about expensive medical charges at the Inpatient Center on the first floor from 8:30 a.m. to 5:00 p.m. on weekdays. Regarding payment for dental care, you can make inquiries at counter No. 7 for payment consultation on the second floor of the Dental Care Center from 8:30 a.m. to 5:00 p.m. on weekdays.

Procedure at the time of discharge

If your discharge from the hospital has been decided, your doctor, dentist or nurse will explain the discharge to you. If there have been any changes in your health insurance status during your stay in the hospital, please let us know.
A nurse will let you know when your hospitalization bill has been prepared. Please pay the bill at an automated cashier machine on the first floor. Credit cards are also accepted.
If you are discharged from the hospital on a Saturday, Sunday or national holiday, you are kindly requested to pay your bill on the weekday immediately before discharge. If your discharge is suddenly decided on a Saturday, Sunday or national holiday, you can pay at the clerical duty room next to the off-hours entrance. Please proceed to the clerical duty room when a nurse lets you know.
